Advance Care Planning via Group Visit

The Veteran Affairs of Palo Alto Health care System (VAPAHCS) and all its community-based outpatient clinics would like to welcome you and your loved ones to our new telephone and soon-in person class. We welcome you to discuss legal documents, mental health treatment details no one likes to think about, and the importance of having difficult conversations with those we assign as emergency contacts also knowns as Health Care Agents (HCA).

So what constitutes an emergency? 

One example of an emergency is any circumstance you cannot speak. Possibly you experience a weird side-effect or have an unknown allergic reaction to medication, insects, or anything in your environment.

Details about what YOU want in an emergency situation that will comfort YOU need to be discussed and not left up to others for many reasons. Emergencies can affect individuals while also testing their emotional intelligence.

What about cultural practices? Pets you may want to have visit? We cannot possibly think of everything but any small detail you include about you in the Living Will section will make all the difference in the world for YOU. The VA wants to help you in those moments by providing educational material to remind you to prepare you and your loved ones.

This all matters because we do not know the future. Our health and mental health are fragile due to us being mortal. We will be discussing a variety of plans while reviewing your medical treatment goals in advance, hence why we call it the Advance Care Planning class. Classes will discuss goals in life and how it relates to goals of care to help you with your Advance Care Planning at all ages. (ACP is not just end of life planning-its planning your life and the chore of what matters to you.

Why make the effort to tell you all this? Well, because we are human, vulnerable, and accidents happen every day that may hurt our mortal bodies.

Bio: Sylvia has been with the VA for a year and a half. Currently, she is working as an Advance Care Planning-Social Worker who facilitates, plans, organizes, and manages group visits for rural Veterans with her amazing team. Sylvia graduated USC in 2017 and has experience with families and children in crisis providing direct interventions in homes and the community. Sylvia also recently experienced working with Sex Offenders to help reduce recidivism by providing specialized group treatment along with individual therapy providing treatment which is (CASOMB) California Sex Offender Management Board approved. Sylvia is also an Air Force Veteran who supported (AFMAO) Air Force Mortuary Affairs Operations during her time in service. Sylvia is currently studying for her Clinical License in Social Work and hopes to one day support a Not-for-profit that helps families with life skills, parenting, and therapy.
